An insurer name on a clinic profile does not guarantee your specific service, provider or plan will be covered. Your plan determines eligibility and reimbursement.
Check the provider's regulated professional title, the exact treatment being booked, any referral requirements, plan maximums and your remaining balance.
A clinic may offer direct billing, but direct billing is a payment convenience, not a guarantee that the insurer will approve the claim.
Request the full appointment fee, ask what happens if direct billing is declined, and clarify whether you must pay first and submit a receipt.
